Targeted Clinical Activities
The Telehomecare web platform allows patients to benefit from remote clinical monitoring and education. From home, patients answer questions about their general condition and health. This information is transmitted to the health care provider in charge of follow-up. The tool enables the patient to be guided in learning about the disease and to interact with their provider when necessary.
More concretely, the Telehomecare platform offers you:
- information and advice tailored to your health situation;
- a messaging function to communicate with your professional;
- a health library with learning materials.
What are the benefits?
This modality offers several advantages for people with chronic illnesses:
- the possibility of gaining greater autonomy in managing their illness;
- a feeling of closeness to the care team;
- improved quality of life;
- fewer emergency room visits and hospitalizations.
Who is it for?
People with the following health conditions can benefit from Telehomecare monitoring (if offered at their facility):
- Chronic Obstructive Pulmonary Disease (COPD)
- Diabetes
- Gestational diabetes
- Heart Failure
- High Blood Pressure (hypertension)
- High-Risk Pregnancy at Home
- Post-surgical of the aortic valve by Ross Procedure
- Eating Disorders in adolescents
- Palliative Care
- Oncology : Patients on oral anticancer therapy (MAVO)
- COVID-19 Symptoms
- Changes in the CHSLD user’s condition
Note: Telehomecare is mainly used to monitor chronic diseases. However, as the list shows, this approach may also be appropriate in other contexts.
Description of the technology
The Telehomecare Platform has a space (called a “portal”) designed for patients. The display of this portal adapts to the device used: computer, tablet or smartphone.
The main functionalities of the patient portal:
- personalized questionnaires based on the person’s health condition;
- a health library;
- a messaging and document-sharing section.
Access to the Platform
If the service is in place in your environment, you can use the platform from your tablet, smartphone or computer.

Users sometimes need support. This may be related to a problem accessing or using the platform. If this is the case, patients should first contact the professional or clinic offering Telehomecare monitoring.
